Informed Consent for Therapeutic Companionship

About Our Sessions

As part of our time together, I will need to collect and hold information about your current circumstances and lived experiences. This helps me offer care that is attuned, responsive, and rooted in your truth. You are not required to share anything that doesn’t feel safe or aligned, but please know that this may affect how I am able to support you. Your participation is entirely voluntary. You may pause, withdraw, or end our work at any time.

 

 

Why I Collect This Information

Your voice, story, and presence are held with reverence. Any personal information gathered during our sessions is used solely to support your healing and liberation. Notes are recorded and retained only by me, Nadine Aateeyah, to ensure continuity and care throughout our time together.

 

 

Record Keeping

In alignment with professional and legal standards, session notes are stored securely and confidentially. These records are kept for a minimum of seven (7) years following our final session. For companions under the age of 18, records are kept until you turn 25.

 

 

Access to Records

You are welcome to request access to any records or notes held about you unless legal limitations apply. We can speak together about the most supportive and clear way for this to happen.

 

 

Confidentiality & Its Limits

Everything you share in our sessions is held with sacred confidentiality. However, there are a few exceptions where I may be required to disclose information:

 

  • If required by a court subpoena

  • If I believe there is a serious and immediate risk of harm to you or others

  • If you provide written permission to share information with another person (i.e. GP, legal representative, or a support person)

  • If disclosure is otherwise required by law

 

 

We will always seek to discuss this with you first wherever possible,

& honour your dignity in the process.

Fees & Payment

  • Individual session || $190 AUD

 

  • Couple session || $235 AUD

 

  • Payment is due 72 hours before the session, via the Calendly Booking System. Concession or community-supported rates may be available upon request, based on availability.

 

Please note: Al–Kīmiyā is an independent, self-determined healing practice. We do not operate under Medicare or private health rebate systems. All sessions are privately billed.

 

 

Cancellations & Refunds

If you need to reschedule or cancel, please provide at least 36 hours’ notice. Late cancellations will incur the full session fee. If an urgent or exceptional situation arises, you are invited to contact us as soon as possible, and we will respond with compassion.

 

Please note that all refund requests incur a $7.50 administrative fee

to cover processing & system charges.

 

 

Your Rights

You are entitled to be treated with respect, safety, and care throughout our time together. We are committed to upholding the ethics of social work practice—including confidentiality, dignity, cultural humility, and social justice.

 

 

Mutual Care & Right Relationship

This space is rooted in presence, dignity, and shared accountability. If at any time something feels unclear, uncomfortable, or out of alignment, I invite you to speak with me, Nadine directly. Our relationship is one of mutual respect, and I am committed to listening and responding with care. If you wish to seek support or offer feedback through external channels, you are always welcome to do so.
